-We will need medical records for ALL new patients prior to your first visit. Please have them emailed to eurekafamilypet@gmail.com
 

-Please fill out the consent form prior to your appointment time! They are sent to your email when you make the appointment and it will save you (and us) time. 
 

-Yearly Wellness visits will need to bring a fresh stool sample (collected within 4 hours of appt or refrigerated overnight, not left outside any any amount of time).

-If your pet is scheduled for surgery: no food or water after 10:00 pm the night before.

 Surgery patients need to be dropped off between 7:30 am and 8:30 am. Please allow 15-20 minutes to confirm necessary info and forms the morning of surgery.

-For the safety of your pets, please make sure they are in a carrier or on a leash!
If you do not have one, we will provide one appropriately for an additional charge.
